Rash around armpits now spreading to leg/crotch area

About 6 months ago, I developed some type of rash around my armpit area. The rash consists of very small, itchy reddish/brownish bumps that appear in a circular pattern around my armpits as opposed to the center of the armpits.

I visited a dermatologist who gave me something that says "triamcinolone acetonide ointment usp 0.1%" on the tube. When applied to the affected area, it seems to make the rash clear up pretty quickly, but only temporarily, then it comes back again within a couple of weeks. This has been going on for about 6 months. I'd go back to a dermatologist, but I don't have health coverage at the moment so that is not an option.

In the last few weeks, it seems as though I am experiencing the same conditions on the inside of one of my thighs (upper thigh as well as the very edge of the crotch area, right where the underwear line is).

The really strange part is that it only really gets itchy at night time when I get into bed. All day long, I am fine (for the most part), but as soon as I get into bed, the itching starts right up immediately. I've tried washing my bedsheets, changing my deodorant, changing my laundry detergent, and changing my body wash, but nothing has helped. And now that it's starting to appear elsewhere on my body, I'm starting to worry about where else it will show up and if it will ever go away.

I am not sure if it makes a difference, but I am a 39 year old male. Does anyone know what could be causing this and how I can make it go away once and for all?
